Description:
Request for proposals are invited for Medical and Life Insurance Services - FY2022/2023 The requirements and scope of services shall comprise of the following as prescribed in the matrix below. Bidder's company profile including company structure and organizational set up Full technical proposal Statement of company experience in providing similar services Latest two years Audited Financial statement for South Sudan operations. Legal documents including valid tax clearance certificate permitting bidder to operate in South Sudan. A list of some of the key Organizations that your company has served over last 4 years that can be reference checked. Cost proposal for Medical cover (both in & out-patients) and Life covers. The two cost proposals should be separated from each other. a. Out-patient coverage for 1800 USD per year per family b. Out-patient coverage for 1500 USD per year per family c. In-patient coverage for 15000 USD per year per family d. In-patient coverage for 10000 USD per year per family List of current hospitals/dinics in South Sudan and outside South Sudan and copies of running agreements/contracts and contacts that the bidder has with the facilities. The proposal should take into consideration the desired minimum medical benefit table outlined below. If the insurance company own hospitals/clinics within South Sudan, please provide the list and names of technical staff who operates them.
